I'm flatting down the gloss pattern coat on this plug with 400 grit and there a few small break throughs and holes in the surface, can i touch these up with pattern coat and re-flat? or maybe just polish out these areas on the mould?

That!
If you touch up bits like that and re-flat you'll end up with a rippled surface. Its nigh on impossible to properly flat touch ups IME. Being a lazy arse I'd be tempted just to polish it up though. If you get a slight change in surface on the mould just run the polisher over that too.

I've continued flatting the plug and it looks like that is the only break through, so I think I'll polish it, if it does show in the the mould I'll polish it out then. There are a few little pin holes caused possibly by bubbles / fisheyes in the pattern coat also.

Think of how much work it has taken to get you this far. If you don't go for perfect you will wish you had later.
In my view Perfect is just about good enough.
